- £25,545 Basic / £29K - £32,500 per annum with on-call

- On-call one or two weeks per month @ £290 per week
- On-call once per month: £29K
- On-call twice per month: £32,500
- On-Call Hours: Monday - Thursday: 17:00 - 22:00 Friday 17:00 - Monday morning

Duties
As a Care Coordinator, you will play a pivotal role in scheduling and managing care rotas for our dedicated team of homecare workers. Key responsibilities include:
- Planning and organizing rotas to ensure continuity and quality of care
- Managing day-to-day scheduling changes and ensuring all shifts are covered
- Liaising with clients and their families to arrange and confirm care schedules
- Monitoring care plans, logging changes, and updating our Staff Plan system
- Providing support and guidance to the care team, responding to queries, and handling emergencies when on call
- Ensuring all regulatory and compliance standards are met in every care plan and visit
Criteria
- Minimum of 2 years’ experience in homecare.
- Proven experience in care coordination and rota planning.
- Car Driver Essential.
- Strong organizational and prioritization skills.
- Proficient with computer systems and scheduling software.
